Medical Consent
This consent form acknowledges that your child is physically capable of participating in in any and all activities prepared by the European Football Academy Okinawa (EFAO).
In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.
I understand that the EFAO is not responsible for accidents or injuries occurring at training, camps, or games. In the event that emergency medical aid/treatment is required due to illness or injury during practices or games, the European Football Academy Okinawa staff will secure medical treatment and transportation if needed.
